What is Heartland Connect (HCoC Connect)?

Heartland Connect is a first-of-its-kind mobile app designed to collect and track street outreach data for people experiencing homelessness and support efforts to address homelessness and housing insecurity in Springfield and Sangamon County. Whether you're someone needing support, a street outreach worker, or a concerned community member, the app simplifies the process of connecting people to services and resources.

With real-time tools and a centralized platform, Heartland Connect empowers users to make a tangible difference. Outreach workers can document engagements, report encampments, and locate critical services, while community members can easily report non-emergency situations and areas of need.

Whether you're looking for shelter, food, healthcare, or just need someone to talk to, the app helps connect individuals to outreach teams and services, while also providing access to over 120 local resources and the HCoC Community Calendar.

Who is HCoC Connect for?

When should I use HCoC Connect?

Use HCoC Connect in non-emergency situations where you or someone else may benefit from outreach support:

  • When you or someone else may be experiencing homelessness
  • When you or someone else might need need outreach or support
  • When a situation is on-going and follow-up could help

How do I submit an Outreach Form?

Helpful Tips:
  • You do not need to engage individuals to complete this form.
  • Provide as much information as possible.
  • Be as accurate as possible.

Step 1: Enter your contact information (name, number, email) and the time, date, and location of interaction. 

Step 2: Share basic information about yourself or the individual to help outreach teams better understand the situation and provide the right support. This may include name (if known), general appearance, possible ways to make contact or frequent locations, and any details about needs or circumstances.

How can I use HCoC Connect respectfully and responsibly?

HCoC Connect is designed to support people and connect them to services when possible. 

  • Do not submit information to harass or target individuals
  • Avoid harmful assumptions or language
  • Share accurate information that is helpful and necessary
HCoC Connect should not be used for emergencies or urgent medical situations. If someone is in immediate danger or in need of immediate medical attention, call 911. 

Do I have to include my contact information?

Yes. A valid email address and phone number is needed when submitting a report. This allows outreach teams to follow up if additional information is needed. 

Your information is kept private and is only used by outreach teams within the Heartland Continuum of Care. We do not sell or share your information publicly, and secure data practices are in place to protect it.

Will I receive updates after I submit an Outreach Form?

If you are submitting for yourself: Outreach teams will use the information you provide to follow up and connect you with ongoing support and services for as long as you want and need.

If you are submitting for someone else: You will have the opportunity to let us know if you would like to be notified when our outreach team has connected with the individual. Due to privacy and confidentiality, detailed updates are not shared outside of the Heartland Continuum of Care.

Can I submit multiple reports for the same person?

Yes. If you see the same individual again, specifically in a different location or situation, additional reports can help outreach teams respond more effectively.
