HCoC Connect is now available for download on both Apple and Android devices.

The app allows community members to quickly and easily connect individuals experiencing homelessness with local outreach teams. Submissions take about a minute and help ensure people are connected to support, resources, and services in real time.

Outreach teams respond directly and work to meet individuals where they are, offering assistance ranging from basic needs to housing navigation and connections to community partners.

This tool was created to make it simple for anyone in the community to take action when they see someone who may need support.
