Heartland HOUSED Q1 2026 Update Highlights Progress, Housing Growth, and System Impact

In 2025, Heartland Continuum of Care partners served more people than ever before, with the number of people accessing services increasing 40% since 2019. During the same year, 409 people exited homelessness into stable housing — the highest number recorded locally.

By-name data also showed a 14% decrease in the number of people actively experiencing homelessness in 2025, with more people exiting homelessness than entering the system in 8 of 12 months.

The report includes updates on expanded street outreach staffing, distribution of 2,068 Narcan kits in Q1, the community’s 100 Day Challenge on homelessness prevention, strategic plan progress, Welcome Home Baskets, the new outreach vehicle, and the HCoC Connect mobile app.
